General Case Management is a foundational service within community mental health and addictions care. Many clients face overlapping challenges related to mental illness, substance use, housing, income, physical health, and limited access to primary care. Without coordinated support, they often cycle through crisis services, emergency departments, and hospital admissions.

The program offers consistent, relationship-based support that helps clients navigate complex systems, stabilize basic needs, and pursue their own recovery goals. By reducing avoidable crisis use and supporting long-term engagement in care, General Case Management enables people to live safely and sustainably in their community.
