Acute Care Case Management

Transitions from inpatient mental health units back into the community are a high-risk period for relapse, crisis, and readmission. The Acute Care Case Management Program supports clients during this critical window by providing timely, coordinated follow-up after discharge from the Brockville General Hospital Inpatient Mental Health Unit.
