Emergency Room Diversion

Community-based support for people who frequently turn to the ER for mental health needs.

The Emergency Room Diversion Program supports individuals whose mental health and substance-related needs lead to repeated emergency department visits, despite the ER being poorly suited to address the underlying issues. Frequent ER use by a small number of people contributes to system congestion, fragmented care, and poor outcomes for both clients and hospitals.

ER Diversion intervenes proactively through community-based assessment, intensive one-on-one support, and warm connections to mental health, addiction, housing, and primary care services. By stabilizing people in the community, the program reduces avoidable emergency visits and hospital admissions, improves continuity of care, and supports stronger recovery outcomes.
