Leeds & Grenville HART Hub

The Leeds Grenville HART Hub responds to high levels of unmet need related to addiction, mental illness, and homelessness in Brockville and surrounding communities.

As part of a provincially funded, referral-based healthcare initiative operated by LLGAMH, the hub provides a coordinated continuum of care that brings treatment, health services, recovery supports, and housing pathways together in one integrated setting. This approach reduces pressure on emergency, inpatient, policing, and shelter systems while supporting sustained recovery and community safety—ensuring equitable access to intensive, recovery-oriented care across the Leeds and Grenville region.
