HART Hub Supportive Housing

HART Hub Supportive Housing provides stable, recovery-focused housing for people who have completed the treatment components of the HART Hub but continue to need structured supports to maintain stability. Without this step, many participants would be at high risk of relapse, homelessness, and return to crisis services.

The program combines substance-free accommodation with ongoing clinical follow-up, case management, and access to mental health, addictions, vocational, and social supports. By extending treatment gains into daily life, supportive housing protects recovery investments, supports successful reintegration into the community, and reduces long-term reliance on emergency, hospital, and justice systems.
